Can't play FIFA12 Empty Can't play FIFA12

Hi, I have an xbox 360 slim which is modded. I just bought it a month ago. I bought FIFA 12 and it works, but i start playing a match, it suddenly says 'this disc is unreadable'. It works fine when i go to training mode, and quick sim and all that. PLEASE HELP!
